I acknowledge that I have read and understood the House Rules and Regulations stated below and SM Investments Corporation (“SMIC”)’s Privacy Policy found at (https://sminvestments.com/privacy-notice).Accordingly, I hereby give my full and absolute consent to SM INVESTMENTS CORPORATION, its officers, employees, authorized personnel and/or service providers to collect, use, store, access or otherwise process the personal data that I may provide herein, whether manually or electronically, such as but not limited to: (1) full name, employee ID number, email address, mobile number, company of SM employees/tenant; (2) full names of the other players/guests/non-SM employees/non-tenants; and (3) Identification Cards, to facilitate my request for court reservation and for security reasons, for the period allowed by law. I hereby warrant further that I was duly authorized by the other players/guest/non-SM employees/non-tenants to disclose their names for the above stated purposes and that they have likewise read and understood the House Rules and Regulations stated below and SMIC’s Privacy Policy.

House Rules and Regulations

1. Only E-com tenants and SM employees are allowed to play in the gym.
2. For security reasons, management will require enlistment of all players in this form. Enlisted players are required to present their company ID upon entry.
3. Players are required to use playing shoes inside the court. Only rubber-soled shoes are allowed within the court area.
4. Outsiders/watchers/cheerers are not allowed except for leagues
5. Players/visitors are responsible for securing their personal belongings. The company will not in any way be responsible/liable for any losses.
6. No eating, drinking, or smoking inside the court. Water jugs must be placed at the bench area.
7. Any damages caused to the facilities during the contract period shall be charged to the players/lessee.
8. The management could provide a shot clock, scoreboard, and scorer, for the basketball players. Referee (league committees) basketball, shuttlecock.
9. The players/lessee will provide their water.
10. Be mindful of references, email notifications, and account numbers.
11. After the approval from the admin, the tenant will have four (4) hours to make payment or the reservation will be canceled.
12. Payment must be submitted into the portal; otherwise, it will be canceled.
13. Players/Lessees can only book 3 days before the reservation.
14. The cut-off time for reservations and inquiries is 4:00 PM.
15. OneE-com security personnel or the gym in charge has the right to call the attention of any individual violating any of the OneE-com house rules and regulations.
16. No refund shall be allowed once paid. However, rescheduling within one month from the time of reservation will be allowed if SMIC Administration has been notified at least 1 day before the scheduled game.
17. No extension of playing hours shall be allowed if there's a prior reservation.
18. Extension of playing hours: Badminton: 1 hour; Basketball/Volleyball: 1 & 1/2 hour.
19. Maximum number of players for Badminton is 6 pax/court/hour; for Basketball & Volleyball 24 pax/one and half hour.
